i) Hand written application to Registrar birth having subject name inclusion of child by the concerned child parent and to be submitted at International Inoculation Centre (HQ) Birth Centre, Mandir Marg.

ii) Affidavit stamp paper on Rs.10/- duly attested by notary public having subject name inclusion of child
