Birth application
form duly filled by head of the family/Relative etc which are available at International
Inoculation Center (HQ) Mandir marg/Palika Suvidha Kendra/Other Concerned Areas Birth
and Death Centers.
The forms shall be
available on the NDMC Website also.
Certificate will be delivered within 7 working days.

2. Fee Structure
|
For first copy of
birth certificate - Free of Cost, For additional copies Rs.20/-
per copy, Search Fee = Rs.7 /-
|

Documents Required
|
|
Following
Documents are Required for Registration of Home Birth with in One Year of its Occurrence:
1. Duly filled birth reporting form by head of the family or relatives.
2. Certificate of Doctor etc. who assist in delivery of child.
Beyond one year of occurrence of birth event the registration will be done after submission
of S.D.M order from local area along with Rs. 15/- as late fee to international inoculation
centre (HQ) Birth Center Mandir Marg
